Simply print the ENROLLMENT APPLICATION and fill it out (front and back) including your payment selection and fax to 313-972-1400 or mail to: DENCAP Dental Plans, 45 E. Milwaukee, Detroit, MI 48202. Completed applications received by the 25th day of the month will ensure coverage effective the first day of the following month.
